An informant either lies, or just plain gets it wrong. So the police bust into a house. A 92-year old woman meets their entrance with gunshots - and a LOT of them. The police shoot and kill her.

They probably were busting in thinking it was going to be a drug dealer packing heat.
They get gun-fire at them. They return fire.
They then find out that they just shot and killed a woman that is about 90 years old. Alone in the house (at least from the sounds of it).
