Software tester side: they didn't have real users test the software!

Until you do this, you can test til the cows come home, and not be able to create scenarios that real users will. Who would have thought to enter a single apostrophe in the last name field?

It boggles the mind thinking about how many databases contain so-called private information, and of those, how many are accessed via the web...
